How It Works

Aripiprazole is a second-generation (atypical) antipsychotic with a unique mechanism — unlike most antipsychotics that block dopamine receptors, aripiprazole is a partial dopamine agonist. It acts like a dopamine stabilizer: reducing dopamine activity where it is too high (reducing psychosis) while maintaining it where it is needed (reducing negative symptoms and side effects). It also partially activates serotonin receptors. This unique profile makes it less likely to cause weight gain, sedation, or movement problems compared to older antipsychotics.

Taken once daily with or without food. Available as tablets, orally disintegrating tablets, liquid solution, and monthly injectable (Abilify Maintena). Lower doses used for depression augmentation.

Aripiprazole is one of the most weight-neutral antipsychotics available. Akathisia (inner restlessness, urge to move) is the most common complaint — tell your provider if you feel unable to sit still.

Important Safety Information

Tardive Dyskinesia: Long-term use of antipsychotics can cause involuntary repetitive movements of the face, tongue, or limbs. Report any unusual movements to your provider immediately.

Elderly Patients with Dementia: Antipsychotics carry a black box warning for increased risk of death in elderly patients with dementia-related psychosis. These medications are not approved for this use.

Metabolic Effects: Many antipsychotics can cause weight gain, elevated blood sugar, and increased cholesterol. Your provider will monitor these regularly.

Pregnancy & Breastfeeding: Discuss risks and benefits with your provider. Do not stop without consultation.

Drug Interactions: Antipsychotics interact with many medications including other CNS depressants, blood pressure medications, and QT-prolonging drugs. Always share your full medication list.

Impulse Control: Rare but reported cases of compulsive behaviors including gambling, eating, shopping, and sexual urges. Contact your provider immediately if you notice new compulsive behaviors.
