How It Works
Lumateperone is a newer second-generation antipsychotic with a uniquely selective and multi-modal mechanism. It simultaneously modulates serotonin, dopamine, and glutamate pathways — but at much lower dopamine D2 receptor occupancy than traditional antipsychotics. This low D2 occupancy is thought to explain its favorable tolerability profile: minimal movement side effects, minimal weight gain, minimal sedation, and minimal metabolic effects. It is FDA-approved for schizophrenia and bipolar depression (both as monotherapy and adjunct), and is one of the best-tolerated antipsychotics currently available.

Taken once daily WITH food. The dose is fixed at 42 mg for all indications — no titration required. Avoid with strong CYP3A4 inhibitors or inducers.

Important Safety Information
Tardive Dyskinesia: Long-term use can cause involuntary repetitive movements of the face, tongue, or limbs. Report any unusual movements to your provider immediately.
Elderly Patients with Dementia: Antipsychotics carry a black box warning for increased risk of death in elderly patients with dementia-related psychosis. Not approved for this use.
Metabolic Effects: Many antipsychotics can cause weight gain, elevated blood sugar, and increased cholesterol. Your provider will monitor these regularly.
