How It Works
Cobenfy is a first-in-class antipsychotic with an entirely novel mechanism — it is the first schizophrenia medication that does NOT block dopamine D2 receptors. Instead, it works by activating muscarinic acetylcholine receptors (M1 and M4) in the brain. Xanomeline activates these receptors to reduce psychotic symptoms through a completely different pathway than all other antipsychotics. Trospium chloride is added to block muscarinic receptors in the body (but not the brain), reducing the peripheral side effects like nausea and sweating that xanomeline alone would cause. This combination represents the first new mechanism of action for schizophrenia in over 60 years.

Taken twice daily WITH food. Start at 50/20 mg twice daily for 2 days, then increase to 100/20 mg twice daily. Must be taken with food to reduce nausea. Do not crush or chew capsules.

Nausea and GI side effects are the most common complaints, especially early in treatment. Always take with food — this significantly reduces nausea. Cobenfy does not cause the movement side effects or tardive dyskinesia associated with traditional antipsychotics because it works through a completely different mechanism.

Important Safety Information
Novel Mechanism — Long-Term Data Still Emerging: Cobenfy was FDA-approved in 2024. While clinical trials showed strong efficacy and tolerability, long-term real-world data is still accumulating. Your provider will monitor you closely.
Bradycardia: Cobenfy can slow heart rate. Tell your provider if you feel faint, unusually tired, or notice a very slow pulse. An EKG may be recommended before starting.
Urinary Retention: The trospium component can cause difficulty urinating. Contact your provider promptly if you have trouble urinating.
Drug Interactions: Trospium interacts with other anticholinergic medications. Avoid combining with other anticholinergics. Share your full medication list.
Pregnancy & Breastfeeding: Limited safety data available as a newer medication. Discuss risks and benefits carefully with your provider.
Elderly Patients with Dementia: Like all antipsychotics, carries a black box warning for increased mortality in elderly patients with dementia-related psychosis.
