How It Works

Valproate is a mood stabilizer and anticonvulsant that works primarily by enhancing the activity of GABA — the brain's main inhibitory neurotransmitter — and by blocking sodium channels in neurons. This combination reduces the excessive neuronal firing associated with seizures and manic episodes. It is one of the most effective treatments for acute mania and mixed episodes, and is often preferred over lithium when rapid cycling, mixed features, or a history of substance use is present. It requires monitoring due to liver toxicity and significant risks in pregnancy.

Important Safety Information

Severe Fetal Harm — BLACK BOX WARNING: Valproate causes major birth defects (neural tube defects, cleft palate, heart defects) and significantly reduces IQ in children exposed in utero. It is contraindicated in pregnancy and in females of childbearing age unless no alternatives exist and REMS program requirements are met. Effective contraception is mandatory.

Liver Toxicity — BLACK BOX WARNING: Rare but potentially fatal liver failure, most often in children under 2. Signs include jaundice (yellowing of skin or eyes), dark urine, severe stomach pain, and fatigue. Seek emergency care immediately.

Pancreatitis — BLACK BOX WARNING: Rare but life-threatening inflammation of the pancreas. Signs include severe stomach or back pain, nausea, and vomiting — seek emergency care immediately.
