Levomilnacipran

Generic name: levomilnacipran hydrochloride  ·  Brand name: Fetzima®
Depression Fatigue Motivation
🧠

How It Works

Levomilnacipran is a serotonin-norepinephrine reuptake inhibitor (SNRI) that is unique in that it preferentially inhibits norepinephrine reuptake more than serotonin — the opposite ratio of most SNRIs. This stronger norepinephrine effect may make it particularly helpful for patients who struggle with low energy, motivation, and concentration alongside depression. It is FDA-approved specifically for major depressive disorder.

What It Treats

Major Depressive Disorder (MDD)FDA-approved; particularly helpful when fatigue, low motivation, and concentration are prominent symptoms
Treatment-Resistant DepressionSometimes used when other antidepressants have not been fully effective
FibromyalgiaUsed off-label; norepinephrine action helps with pain modulation

Important Safety Information

⚠️
Black Box Warning (under 25): Antidepressants may increase risk of suicidal thinking in children, adolescents, and young adults, especially in the first few weeks. Monitor closely and contact us right away if new or worsening thoughts of self-harm appear.
🔴
Serotonin Syndrome: A rare but serious reaction when combined with MAOIs, triptans, tramadol, or St. John's Wort. Signs include fever, agitation, rapid heart rate, and muscle twitching — seek emergency care immediately.
💊
Drug Interactions: Do not take with MAOIs (must wait 14 days between). Use caution with blood thinners, NSAIDs, and other serotonergic medications. Always share your full medication list.
🩸
Blood Pressure: SNRIs can raise blood pressure, especially at higher doses. Your provider may monitor your blood pressure periodically.
🤰
Pregnancy & Breastfeeding: Discuss risks and benefits with your provider. Do not stop without consultation.
Discontinuation Syndrome: SNRIs — especially Effexor — can cause significant withdrawal symptoms if stopped abruptly: dizziness, brain zaps, nausea, and flu-like symptoms. Always taper slowly with your provider's guidance.
🚽
Urinary Retention: Levomilnacipran can cause difficulty urinating, especially in men. Contact your provider right away if you have trouble urinating or feel like your bladder is not emptying fully.
💓
Increased Heart Rate: Fetzima raises heart rate more than other SNRIs. Your provider may monitor your pulse and blood pressure, especially early in treatment or after dose increases.
