How It Works
Lybalvi is a combination tablet containing olanzapine and samidorphan. Olanzapine is a well-established, highly effective antipsychotic — but it is known for causing significant weight gain. Samidorphan is an opioid receptor antagonist added specifically to reduce the weight gain associated with olanzapine. Clinical trials showed Lybalvi caused significantly less weight gain than olanzapine alone while maintaining the same antipsychotic efficacy. It is a good option for patients who have responded well to olanzapine but are concerned about weight or have experienced significant weight gain on it.

Taken once daily with or without food. The samidorphan component is always 10 mg regardless of olanzapine dose. Cannot be used in patients currently using opioids or those in opioid withdrawal.

Lybalvi provides the proven effectiveness of olanzapine with meaningfully less weight gain. However, weight gain can still occur — it is reduced, not eliminated. Metabolic monitoring is still important.

Important Safety Information
Tardive Dyskinesia: Long-term use can cause involuntary repetitive movements of the face, tongue, or limbs. Report any unusual movements to your provider immediately.
Elderly Patients with Dementia: Antipsychotics carry a black box warning for increased risk of death in elderly patients with dementia-related psychosis. Not approved for this use.
Metabolic Effects: Many antipsychotics can cause weight gain, elevated blood sugar, and increased cholesterol. Your provider will monitor these regularly.
Pregnancy & Breastfeeding: Discuss risks and benefits with your provider. Do not stop without consultation.
Drug Interactions: Antipsychotics interact with many medications. Always share your full medication list.
Opioid Contraindication: Lybalvi contains samidorphan, an opioid antagonist. It is contraindicated in patients using opioids (including opioid pain medications) and in patients undergoing opioid withdrawal. Using opioids with Lybalvi may precipitate withdrawal and will reduce opioid effectiveness.
Diabetes Risk: Like olanzapine, Lybalvi can elevate blood sugar and increase diabetes risk. Monitor blood glucose regularly.
