How It Works
Brexpiprazole is a second-generation antipsychotic and serotonin-dopamine activity modulator (SDAM) — closely related to aripiprazole. Like aripiprazole, it is a partial dopamine agonist (stabilizer) rather than a full blocker, and also partially activates serotonin receptors. This unique profile provides antipsychotic and antidepressant effects with generally fewer side effects than older antipsychotics. It is FDA-approved for schizophrenia, as an adjunct for major depression, and most recently for agitation associated with Alzheimer's dementia.

Taken once daily with or without food. Start low and titrate slowly. CYP2D6 and CYP3A4 status may affect dosing — your provider may adjust based on other medications.

Brexpiprazole is generally well tolerated with a lower risk of movement side effects than many antipsychotics. Weight gain and akathisia are the most common concerns. Used at lower doses when added to antidepressants.

Important Safety Information
Tardive Dyskinesia: Long-term use of antipsychotics can cause involuntary repetitive movements of the face, tongue, or limbs. Report any unusual movements to your provider immediately.
Elderly Patients with Dementia: Antipsychotics carry a black box warning for increased risk of death in elderly patients with dementia-related psychosis. These medications are not approved for this use.
Metabolic Effects: Many antipsychotics can cause weight gain, elevated blood sugar, and increased cholesterol. Your provider will monitor these regularly.
Pregnancy & Breastfeeding: Discuss risks and benefits with your provider. Do not stop without consultation.
Drug Interactions: Antipsychotics interact with many medications including other CNS depressants, blood pressure medications, and QT-prolonging drugs. Always share your full medication list.
Impulse Control: As with aripiprazole, rare cases of compulsive behaviors (gambling, eating, shopping, sexual) have been reported. Contact your provider immediately if you notice new compulsive urges.
