How It Works
Cariprazine is a second-generation antipsychotic and partial dopamine D2/D3 receptor agonist. Its unique feature is its particularly strong activity at dopamine D3 receptors — which are thought to be involved in negative symptoms of schizophrenia (flat affect, social withdrawal, lack of motivation) and cognitive function. This makes it potentially superior to other antipsychotics for treating the negative symptoms that are often most disabling and hardest to treat. It is also FDA-approved for bipolar depression, making it a versatile option.

Taken once daily with or without food. Has a very long half-life — side effects may appear or worsen for several weeks after a dose change. Titrate slowly and allow time between dose adjustments.

Akathisia is the most common and significant complaint with cariprazine. Because of its very long half-life, side effects can linger for weeks after a dose change. Report restlessness or inability to sit still to your provider promptly.

Important Safety Information
Tardive Dyskinesia: Long-term use of antipsychotics can cause involuntary repetitive movements of the face, tongue, or limbs. Report any unusual movements to your provider immediately.
Elderly Patients with Dementia: Antipsychotics carry a black box warning for increased risk of death in elderly patients with dementia-related psychosis. These medications are not approved for this use.
Metabolic Effects: Many antipsychotics can cause weight gain, elevated blood sugar, and increased cholesterol. Your provider will monitor these regularly.
Pregnancy & Breastfeeding: Discuss risks and benefits with your provider. Do not stop without consultation.
Drug Interactions: Antipsychotics interact with many medications including other CNS depressants, blood pressure medications, and QT-prolonging drugs. Always share your full medication list.
Akathisia Risk: Cariprazine has a higher rate of akathisia (inner restlessness, urge to move) than many other antipsychotics. This is manageable — tell your provider right away so the dose or medication can be adjusted.
